COVID-19 Testing Refund and Cancellation Policy

YOUR RESPONSIBILITY:
– All patients purchasing COVID-19 tests must use their own judgment regarding the best date/time to have their test to meet the entry requirements of their travel destination.
– PRAXES can not advise on, and does not take any responsibility for, any test timings and their associated outcomes including (but not limited to) the effect on travel, business or personal plans.
– We encourage patients to check with their airline carrier as well, regarding requirements.
– We recommend not purchasing a COVID-19 test until your travel arrangements are confirmed.

APPOINTMENT CHANGES:
– There is no charge to change the date, time or location of your appointment.
– New appointment times are subject to availability.
– There are NO refunds available for rescheduled appointments within a 72 hour timeframe before your appointment start date/time.

CANCELLATION OF YOUR APPOINTMENT WITH LESS THAN 24 HOURS NOTICE:
– There are no refunds for appointments cancelled with less than 24 hours notice.
– There are NO refunds available for rescheduled appointments within a 24 hour timeframe before your appointment start date/time.
– You acknowledge that you cancel your appointment and forfeit your paid amount entirely if you are later than 15 minutes for your appointment.

CANCELLATION OF YOUR APPOINTMENT WITH 24 to 72 HOURS NOTICE:
A $25 + HST administrative fee will be applied to your paid amount upon refund for appointments canceled between 24 to 72 hours before your scheduled appointment.
– There are NO refunds available for rescheduled appointments within a 72 hour timeframe before your appointment start date/time.

CANCELLATION OF YOUR APPOINTMENT WITH GREATER THAN 72 HOURS NOTICE:
A full refund will be issued for the fee charged if you cancel your appointment with greater than 72 hours notice in advance of your appointment.

REQUEST A REFUND AFTER A COVID-19 TEST HAS BEEN COMPLETED:
Once a swab has been performed, it is non-refundable.
